Neuro Diagnostics is committed to protecting your privacy and safeguarding your personal health information. We follow the Personal Health Information Protection Act (PHIPPA) and recognized best practices to ensure your information remains confidential, accurate, and secure.

Your Rights

As a patient or client, you have the right to:

  • Access the personal health information we hold about you

  • Request corrections or updates to your health record

  • Be informed about how your information is collected, used, and shared

  • Withdraw or limit consent for the sharing of your health information (see Consent Directives below)

Our Practices

We collect, use, and disclose personal health information to:

  • Provide and coordinate your healthcare and diagnostic services

  • Communicate with you about appointments, test results, or follow-ups

  • Support administrative, billing, and quality-improvement functions

  • Meet legal and regulatory requirements under PHIPPA

We do not sell or share your personal or contact information for marketing or SMS purposes.

Your Health Record

Your health record includes:

  • Contact details and demographic information

  • Health history and family health history

  • Records of visits, care provided, and test results

  • Information from other healthcare providers involved in your care

You may request access to your health record or ask for a copy of it. In rare cases, access may be restricted in accordance with PHIPPA. If you believe there is an error in your record, you may request a correction or submit a statement of disagreement to be attached to your file.

Consent Directives / Lockbox

You may ask Neuro Diagnostics not to share some or all of your health record with specific members of our care team or external healthcare providers. This is known as a consent directive or “lockbox.”

Our team will explain how such a request may affect your care. In limited circumstances, we may be required by law to disclose restricted information if it is necessary to prevent or reduce a serious risk of harm.
